I don`t mean to blow anyone`s bubble here and I may be wrong But they ask for too much info for just your name I could see them asking for your e mail address and that should be enough but your street house number town and the rest well you may as well just go out and paint an x on your door.

It is a noble thing to stand up, but the people who started this petition are unknown to me and might very well be the people we are resisting . And though yes they could get the information just from publishing your email address it would take them time to find it and track you down instead of you volunteering it .
