Here is a good overview of Hacking Groups employed by Governments to do their dirty deeds. Usually called APT Groups (advanced persistant thread).
Unfortunally the USA is missing in this Google Doc, but it´s quite accurate and is updated regulary.
Sorted after States and their own nicknames and operations.
The last big attack was on the opening ceremony of the olympic wintergames, where the Internet was widely cut off and several Screens went black.
